May Exam Application
Introduction
For administrative purposes, HTCC must require that the candidate personally complete this application.

Applications submitted on behalf of another individual cannot be accepted. In the application, HTCC requires the attestation of accuracy and truthfulness of all responses, including responses to questions about sensitive personal information.

HTCC also requires acknowledgement that its rules and Disciplinary Policy have been read, understood, and accepted personally by each candidate.

Submission of the application demands verification that the preparer of the application be the candidate or certificant himself or herself.
